- Report between 7:30 a.m. and 4:30 p.m. Monday through Friday in appropriate attire. No hats, sunglasses, revealing clothing, or exposed undergarments. No cell phones. No pets, food or drinks are allowed in lobby. Family must wait outside.
- Do not report more than a week before your mandatory report date. If you do so, you may be asked to come back at a later date.
- Bring $90.00 for the process fee. US currency, US Postal money order, or credit/debit card (Visa or Master Card) are accepted.
- Bring one of the following types of photo identification: Valid DMV Identification Card or Driver’s License, valid federal or state governmental agency identification card, foreign consulate identification, or passport.
- If you are physically impaired or cannot perform manual labor, you must bring a note from your doctor. Notification by fax will not be accepted.
NOTE: If you are ordered to report to the Blythe Office, please call (760) 863-8658 for an enrollment appointment.
